Fall sports are here, and we want to ensure all student-athletes are fully prepared to participate. Please make sure your student-athlete is registered in Final Forms for their fall sport, all forms are completed, and their physical is current. Failure to complete registration, forms, and provide a valid physical will result in removal from participation until everything is submitted.

Final Forms Registration Instructions:

  1. Visit: https://talawanda-oh.finalforms.com/
  2. Log in to your parent account.
  3. Click on the "PROFILE" button for the student you would like to register for a sport.
  4. Click on the "MANAGE SPORTS" button below the student’s profile information.
  5. Select the sport the student would like to participate in this season.
  6. Click the "UPDATE SPORTS" button at the bottom of the page to complete registration.

Important: While logged in, please ensure that all forms are complete and that an OHSAA Preparticipation Physical is uploaded.  FORMS MUST BE COMPLETED BY BOTH THE PARENT AND STUDENT-ATHLETE TO START PARTICIPATION on August 1st.

Physicals Are Required Before Participation

  • Athletes must have a physical completed and uploaded in Final Forms.
  • No participation in practices or tryouts will be allowed until a valid physical is on file starting August 1st.
  • The OHSAA physical form can be found here.

Final Forms registration ends for all Fall sports on Friday, August 1st, 2025. If you are not registered by that date, you will be required to seek approval from the head coach of the sport you wish to participate in and register in person by visiting the Athletic Office.

If you would like more information on any of the fall sports, please reach out to the appropriate head coach, whose contact information is listed below:
